The banner program has been put on hold until next Spring. We will begin placing banners at the end of April/May. Feel free to submit an application. No money is needed at this time. When we begin printing banners we will reach out to you. We will be making some improvements to the program over the course of the winter. Thank you for your understanding.
Veteran Recognition Banner Application
Complete the application form in its entirety. We will be asking for the following information:
- Service person’s name.
- A high resolution digital image (minimum 300 dpi) in jpeg format photo of the honoree in official uniform (no objects).
- Branch of service: Army, Navy, Air Force, Marines, Coast Guard, National Guard, Space Force.
- Proof of Pana vicinity residency for service person.
- Verification of active duty dates (i.e. copy of DD214).

By submitting the application you hereby grant the Pana Military Banner Project coordinators to use the attached documents (which includes a likeness of me, my relative or person being sponsored) in the Pana Military Banners Project without payment or other consideration.
For security reasons we suggest that you copy your DD214 mark out the social security numbers, and send us a scan of the copied and marked out document. Reach out to Keith Cearlock for other options if you prefer not to upload your DD214.

The process to obtain a free DD214 may take several months. Because Of this we will waive the immediate requirement for the DD214 so we can get the banner up.
